Licence Information

• Commitment Form - To be returned with Schedule 4 and Data Order Form
• Schedule 4 - 2 copies to be completed by ALL participating Sites and returned to Chest with a Commitment to Participate Form, and Data Order Form
• Data Order Form - now incorporated in the licence form download
• Updates Order Form  - this order form should be printed and sent to Chest together with a purchase order to cover the cost of media for licensed sites ordering upgrades to data already licensed via Chest
• Modification Form - two copies to be used to add/delete products licensed, to extend licence period and/or to change contact details
• Appendix A - General Licence Conditions
