Licence Form (Schedule 4) (.rtf file) (for 1st October 2005 to 30th September 2008). Please download and return 2 completed licences to Chest, together with a purchase order.

ADDITIONAL INFORMATION

Addendum Form(.rtf file) (for 1st October 2005 to 30th September 2008) - if your Institution is already licensed for one or more products and wishes to add another product please download 2 copies and return to Chest, together with a purchase order.

Business Source Complete request form- please complete and send with your licences if you are interested in subscribing to the new Business Source Complete database.

Location and Use Permissions- a link into the Overview.

Licence Conditions(.rtf) - this covers Appendix A to Schedule 4, the general licence conditions, Appendix B, the copyright acknowledgement form and Appendix C, special conditions for this Agreement.
