• Licence Form (Schedule 4) (.rtf file) (for 1 October 2005 to 30 September 2008). Please download and return 2 completed licences to Chest, together with a purchase order.
• Addendum Form (.rtf file) (for 1 October 2005 to 30 September 2008) - if your Institution is already licensed for one or more products and wishes to add another product please download 2 copies and return to Chest, together with a purchase order.
