• Schedule 4 - two copies to be downloaded, completed and returned to Eduserv along with a Technical Contacts Details Form (see below) and a Purchase Order for the full fees made payable to "Eduserv".
• Technical Contacts Details Form - one copy to be downloaded, completed and returned to Eduserv along with two Schedule 4s (see above) and a Purchase Order for the full fees made payable to "Eduserv".
• Change/Addition of Technical Contacts Details Form - for currently licensed sites to add or change Technical Contact details.
